  2. I got my green card in December in my new married last name, but my passport is still in my maiden name. Will I have issues leaving the US? We have a long weekend in the Bahamas coming up this month and we are worried since they don't match. Of course we will bring the marriage certificate too but after everything we have been through we are scared to leave hahaha! Can anyone confirm that we won't have any issues?
  3. But I already sent off my AOS without the I-693, our interview is in a few weeks so I will just take the sealed envelope with us to the interview?
  4. Where would I see if my medical is shown as complete? At the medical they told me it was all signed off and at the embassy there was no issues - the medical people just told me that I just needed to have the vaccination before doing the AOS, which I did. Ok so I will just take the i693 with me to the doctors and have him fill it out? I don't need to fill out anything? thanks!
  5. Hi all!! Hoping for a bit of advice as I can't seem to find anything anywhere about this! I had my medical at the embassy medical place in London in January 2021, I needed the TDAP booster but they offered it there for like $200 so they said I can just get it at my GP for free. So the next day I went to my local doctors office and got the TDAP booster for free. I have since moved to the US and we filed for AOS in July and we have the AOS interview coming up on December 20th. Since my medical was done within 12 months I know I don't need to redo it, but do I need to get my TDAP vaccine signed off anywhere? I have my vaccination record printed out form my GP with TDAP and COVID on there, I also got the flu shot here a few weeks ago. I have called about 6 of the doctors on the USCIS website and asked if they can just sign off my vaccinations but they are all saying they don't do that since they didn't give the vaccines and that they can only do the full medical? I have an appointment on Monday with one of the USCIS doctors who said he'd take a look at my form and try to help me but I want to know specifically what parts of the i693 he would need to fill out/do for me. Also - does it have to be in a sealed envelope?? Do I have to sign anything?? I'm not sure if I'm missing something or doing something wrong here but if anyone can advise that would be great!
