Timeline

Hello Friends. We are in the process of filling out the I 485 (AOS) and realized that in one of our sample cover letters it says to submit:

- Form DS-3025 (vaccination worksheet) from my K1 visa medical, conducted in ( Germany)

- Form I-693 Appendix A (vaccination supplement) signed by MD in (Happytime USA) in a sealed envelope

Our question is do we have to submit BOTH forms DS 3025 AND I693???

We called the uscis but they had not definite answer. Basically they said 1) submit ONLY the DS3025 and if they write us an RFE requesting I693 then later send it (which means slowing the process) 2) go to a civil surgeon and have them fill out the i693 if the MD says it is required (which we just found out takes 2-3 weeks to process)

Our confusion is that both the DS3025 he obtained signed for in Germany is almost IDENTICAL to the I 693 therefore why would they need both??

HAS ANYONE ever submitted JUST the DS 3025 with the AOS or do most people submit both? What did you y'all do.

Your input and advise would be greatly, greatly appreciated. Thanks!!

Why the double posting?

You only need to submit the I-693A with the I-485 when filing to Adjustment of status from a K-Visa.

I-693A took us 1 day to get, we visited the Civil Surgeon, and he transfered the info to the form in 5 minutes time.

Call Dr L E Arnold and contact her office( they normally do not answer the phone, but within hour someone calls you back at your telephone number and you tell them that you need a I693A transferred from DS3025 and they will know what you mean and then tell you exactly what to mail them, also when you get it back do not open the sealed envelope from Dr L E Arnold, that is copy you include in your AOS application and has to be in sealed envelope by US Civil Surgeon Dr to be accepted, Dr Arnold will also provide exact copy of what is in side the envelope for your records also) she will perform the I693A form in about 2 weeks and do it all by mail, and only charge $10 to transfer his DS3025 information onto the I693A, it is better to do this so you do not have a RFE slow you down later. See Below :thumbs:

some people just enclosed the ds3025 from the medical and didnt get an rfe, some did get one! i think it totally depends which officer will work on your aos application!

to be on the safe side i recommend getting the I693a from a registered civil surgeon. as most civil surgeons try to rip you off by making you pay about $150-250 for a new medical, which you definately do not need as long as your medical is not older than one year, i just give you the adress of a civil surgeon in texas. she will transfere your info to the i639a for just $10 and it will take about 1 week to get it back in the mail (in a sealed envelope).

just call the office, they will let you know what info you need to send them. and if you just get the answering machine, leave a message with your name and number and they will get back to you.

We got our I-693A transcribed by a Civil Surgeon on the USCIS list of Civil Surgeons for $25.

We didn't submit the one we got in Canada... because we wanted to avoid getting an RFE... I-693A is the U.S. version.

Just hope you don't get an RFE for the full medical :(

I think that is a good question soch. I can tell you what happened in our AOS experience. We did the medical in Thailand for the K1. We also took vaccinations in Thailand (cheaper), because my wife had no record. The doctor in Thailand filled out the DS-3025 and gave us a copy of that form. He then sealed up the original DS-3025 in the envelope along with all the other docs he filled out. A few months later, when we were completing our AOS paperwork, I had a civil surgeon copy the information from the DS-3025 to the I-693a, and sent it in with our AOS package.

When we had our AOS inteview, the IO said, "I already had the DS-3025 from your K1 Medical, so you didn't need to do the I-693a." I was surprised to hear this from her, because I was sure that we had to complete the information on the correct form, which is the I-693a.

Our interview was in Detroit, so I don't know if it will be the same experience in your situation. However, it wasn't a big deal to get the I-693a done. We used Dr. Arnold in Texas. He is popular on this board, and it only cost $10, and took them less than a week to complete it through the post office snail mail. I faxed him the information (DS-3025, name, etc), and they snail mailed us back two copies. One copy was for our record, and the other was in a sealed envelope for the AOS package.

I've heard some people on here say that the DS-3025 was enough at their AOS interview, and I've heard others say that they got RFEs for the DS-3025. So, perhaps better is 'safe than sorry'...which is why I went ahead and got the I-693A.

We also submitted only the DS-3025 for AOS and she was approved without interview, even though she was missing one of the "required" vaccinations (this missing vaccine was clearly shown on the DS-3025).

So, my advice is just send it in with the DS-3025 if you can afford the risk of a small delay IF they send you an RFE for the I-693A.

ETA: I suspect that they are so busy they don't want to make extra work for missing vaccinations so they are just approving them if everything else is in order.

Interesting! We decided to send the DS3025 to Dr. Arnold and not take the risk of an RFE. It is so frustrating that they (uscis) can't even answer this question. Thanks for all your help VJ!!!!
