I-485 and I-693 Question AOS K-1

My wife had her medical done in her home country In June 2020 but didn't get vaccinations because of her strong religious belief. The only thing we got from her doctor in her home country is form DS-3025 (which shows no vaccines received) and a CD of her chest x-ray. She is now here with me in the USA.

 

We will be filing a religious exemption for the vaccine.

 

My question is, when we mail I-485 and I-864 next month, do we have to include the DS-3025, which shows she was NOT vaccinated?

 

I read on the instructions of I-485 and it says:

 

 

You are NOT required to submit Form I-693 at the time you file your adjustment application, but may do so if you wish. Because of the time-limited validity of Form I-693, you may choose to submit your Form I-693 after you file your Form I-485. You may also submit Form I-693 in person at an interview in a USCIS field office, if an interview is required. By waiting to submit Form I-693, you may avoid having to repeat the immigration medical examination.

Don't include the DS-3025.   You take the I-693 to the interview.

August 6, 2021 N-400 submitted by mail

September 7, 2021 I-751 Interview, Sept 8 Approved and Card Being Produced

October 21, 2021 N-400 Biometrics Completed  

November 30,2021  Interview, Approval and Oath

December 10, 2021 US Passport Issued

August 12, 2022 PHL Dual Nationality Re-established & Passport Approved 

April 6,2023 Legally Separated - Oh well

My wife and I recently submitted the 485 with a fully completed and correct DS-3025 and USCIS still sent an I-693 "deficiency" letter saying we would need to bring a 693 to the interview (which I disagree with, but that's not my point here), so given that your 3025 does not show any vaccinations, there's no advantage to including it with your 485.  You'll receive a yellow "deficiency" letter like we did, and you'll just take your 693 with you to your interveiw.
