I-90 and location where you applied for and were granted AoS

Hello everyone,

 

I have a question regarding form I-90, Part 3. Questions 1 and 2 ask the following:

 

1) Location where you applied for your immigrant visa or adjustment of status.

2) Location where your immigrant visa was issued or USCIS office where you were granted adjustment of status. 

 

I'm finding conflicting information online. Some sources say to list the US city you were living in at the time. Others say to include the Service Center based on the 3 letter code on the back of the GC (for example, EAC, LIN, SRC).

 

Does anyone know which is correct?

1) if you got immigration visa outside united states , you state the consulate location. if you did AOS than its the location of your I-485 filing

 

2) same as 1 for immigrant visa, USCIS office will be on the approval notice for I-485
