Your wife's birth certificate looks like an old version. This seems to be issued when the government and city corporation were just starting to implement the standardized birth certificate issuance and registration. The newer formats have the city corporation seal and signature/seals of the registrar and an authorized person. The birth certificate's authenticity cannot also be verified as those ones were not entered in the central database. I would suggest you issue a new birth certificate for your wife (with the original registration date) from the city corporation.

Once its issued, you can also check its authenticity from the government's website using the following link:
http://bris.lgd.gov.bd/pub/?pg=verify_br

Also, i'm quite certain you won't be able to find you wife's information using her current birth certificate.

I also wanted to let you know that if you do decide to get a new birth certificate for your wife, don't notarize it using the notary public. Only photocopies require notarization. The original document should never be notarized. Original documents should be left untouched after they have been issued by the originating authorities.
