I have my Vaccination Records for the I-485 but I'm not sure they will accept them

I-130 filed: February 5, 2016

NOA1: February 8, 2016 Nebraska

NOA2: July 21, 2016

POE: December 19, 2016 Las Vegas

N-400 filed: September 30, 2019

Interview: March 22, 2021 Seattle

Oath: March 22, 2021 COVID-style same-day oath

 

Now a US citizen!

You can include but the ds3025 was most likely included in your medical results which is what they require

It was likely in the sealed packet provided at POE.

By medical file,  I meant the one inside the "do not open " envelope handed over at entry

If they have it, then they have what they need.   Nothing additional required from you to include in the application

@payxibka 

 

I am facing a similar issue, my (now) husband provided a file at the time of entry at immigration when he entered the US on his K1 visa. Now that we are applying for his adjustment of status, we do not have any medical records as those were in the sealed file (including I am assuming the DS3025). Should we just assume that when we file the I-485, USCIS will also get access to my husband's medical records (handed at the time of entry)?

The file your Husband surrendered at point of entry is the sealed packet from the Embassy, he would have gotten that after his interview in London, 

 

On the day of his medical he would have received 2 documents, 1 was a sealed document that he would have surrendered to the embassy in London when he has his interview, the 2nd document was the DS3025, its only 2 pages, i would have him double check all of his paperwork he brought from the UK, the medical facility in London would have stapled a notice to the DS3025 stating do not lose this document, it is needed later for AoS. ( they did with mine )

 

When you file the I-485, you need to send a copy of that DS3025 with it, as you have to take the original with you to your interview at a later date. ( i had my interview today, and i had to have mine with me )

If your husband cannot find the DS3025 which should have been given to him by Knightsbridge on the day of his medical, you could try and see if they would be willing to email another copy to you for record keeping.
