What to enter under - Have you ever been issued a U.S. Visa in DS-160

Hi, 

 

Could someone help me on the question: Have you ever been issued a U.S. Visa in DS-160, as I have multiple visas issued previously- below :

  • Valid Visitor visa (Validity until 2022)
  • H4 visa (No longer valid)
  • F1 visa (No longer valid)

 

There is no option in DS-160, to add another visa. So I'm unsure what to enter in this section?
