First things first.  You need to wait until you get your NOA2.  After that then your packet will go to the NVC for processing.  Sad to say but you are still looking at another month or so before you can do your medical.

Is it okay to start emailing them for the case number as early as 1 week upon receiving NOA2?

Its best to wait 10 working days then you should email them

Yes ... that is a road well traveled around here .. so we all understand.       Once the case file gets to Manila everything will move much faster.

Does my fiance need to send hard copy documents or is it okay to have it all scanned and be sent to me  Are copies of noa1 and noa2 needed?

NOA1 and NOA2 are not needed. Scanned copied of the documents from your fiance are fine (I-134, tax docs, etc.)  Your docs have to be originals (NBI, CENOMAR, Birth Certificate, etc.)
