Timeline

Once You explained, and i took a look at examples, i think it's DS3025 (as the copy they gave is awful), corners of the pages (where it says DS and edition) are left out. BUT it looks close to DS3025 as it's marked as "Complete" and signed, just let me check and i'll definitely get back and let you guys know. I'm almost sure, that there is no pictures of her on the form, hence the terrible copy.

Can I jump on here? My fiancé is coming to America on feb 3rd. He had his medical but did not get a flu shot as there were and still are none available around him (or at the doctor where he had the medical). The doctor said he cannot provide the DS-3025 because he’s missing the flu shot. He will try to call around today and if he can get it and send the doctor that proof he will email him the DS-3025, but if not, what does he do? Go to a civil surgeon in the US and just hope they still have flu shots here? And he won’t be able to file AOS until that’s completed? This is super annoying.

In a nutshell. Yup. He'll have to get his medical transcribed, get flu shot done and THEN they'll issue i-693 here, not DS-3025 (as someone mentioned, only can be issued abroad).
Our doctor was nice enough, and told her to run across the street and get the flu shot done now and then to get medical complete. Literally, during medical she ran out to pharmacy/hospital got flu shot, and came back to finish medical. 
They WILL have flu shot here definitely. Walgreens, CVS, family docs, anywhere. But in the end he'll have to go to civil surgeon to get his shot documented properly. Not sure if you can do the email thing. His medical results are in the package he's bringing for border too, so it's not gonna line up when You send AoS and what they have, and i think it would make more issues (NOT SURE ON THIS ONE, MAYBE SOMEONE WILL CHIME IN).

Ugh this sucks. Trust me he tried to get the flu shot. No one had it. His doctor told him there was an issue with the supplier. I guess he’ll just do it all over again here. Just hope it doesn’t delay it too much. His sister is getting married and he needs to be able to get back there this summer. Ahhhhh

i'm not sure if you REALLY need a whole new medical. I think he might need just vaccinations transcribed to i-693 aand add the Influenza. Not 100% on that.

I'd love to hear what others think on this. It is so confusing. Someone else told me that vaccines are not actually required for K1, so the medical form can be completed overseas without them being complete (which was obviously the case) and you have to get the vaccines on your own. My fiance called the doctor today who did the exam and they said they would email him a copy of the DS-3025 if he can show them proof of flu vaccine, so it seems to me they are two totally separate things. But, again, I don't know for sure and would love to know how I can find out.

You technically don't need vaccines to get visa and ENTER on K-1, but you do however, need them to adjust status. And usually, it's cheaper to get it done anywhere but USA, that's why everyone is pushing to get it done before moving to US, and preferably before Embassy medical, to avoid the hassle, extra costs and time.

I have a question related to this..

 

My fiance's doctor did not give him back the DS-3025 form either (he had his medical in Melbourne, Australia last week). It is not currently flu season there, so they did not give him a flu shot. However, he is not returning to the U.S. on his K1 until May (after flu season here). 

 

So..should the lack of flu shot affect receiving his DS-3025 or not? I had assumed previously that he would just have a waiver for that particular requirement. He is calling tomorrow to find out why he was not given back the form but it seems silly to me that the flu shot would have prevented it in his case, as flu season is March - October there, from what I recall. 

 

Lastly, if the issue IS the flu vaccine, would he be able to get the shot in March/April whenever it becomes available and then the doctor in Melbourne would be able to give him the filled out DS-3025?

Hopefully what I said makes sense. Australia having opposite seasons to U.S. makes it a bit..odd. Haha

Not sure. The doctor should've given more info, or ask them whether he needs the shot or not. DS-3025 has to be signed as Complete to be valid with AoS, not only given. i believe they follow current season in USA (as he/she is immigrating here within 6months, assuming approval and issue of the visa) so i'd say they are going by the time, that the interview will take place, not actual arrival.

The medical clinic should have given him a copy the vaccination form. Call and request a copy to be sent to him   The clinic administers vaccinations according to US CDC requirements so if the flu shot was needed he would have been required. It is to be accurate on the date it is completed

The odd thing is they did not bring up the flu shot at all. He remembers filling out the top part of the DS-3025 form (with his name, etc) but they did not mention anything about completion and he did not know he was supposed to be given something back.

Which clinic ? William St Melbourne or Bridge rd Richmond ? Has he picked up the sealed envelope or are they sending it direct to the Consulate
