AoS I haven't applied for SSN and my I-94 expires in 10 days

But that is talking about the DS-230 when applying for the visa, not the I-765 when applying for adjustment of status?

OP you can tick the box yes if you'd like. You may get lucky and receive an SSN that way. It is a option for the newly updated form which was designed to attempt to save time and effort. However, like everything it isn't always reliable. You can find some threads on this forum where it worked for a few people and some where it didn't work at all and they had to go in to the office anyway. Which is precisely what a uscis faq sheet on this option says. It could work, or it might not, and you'll just have to go apply in person. Personally I like dealing with the SSA directly, instead of relying upon a different agency to share info or have it fall through the cracks. If it doesn't arrive within a week or so of receiving the EAD, expect to go to the SSA as normal procedure.

No.  The name change is a separate form on the SSN's website, as it is unrelated to immigration.
