The medical staff can determine by interview with you and with skin observation and blood work which vaccines you will need. So it is ok to express to them that you don't have a record. It's done all the time.

Also you can call other medical facilities and ask if they do immunization shots for the K1 visa,  they will give you bloodwork etc like Greenbaum said to see which ones you need and write it in a little booklet.  This will be less expensive than getting them during your K1 medical exam. Some shots can't all be given at once depending on what you need and you'll have to come back for the rest it might add more time before your interview date also some shots can wait and be given in the US also more expensive.  So if the option is available to you get them done before you medical.

@Tidus992 really? I thought the k1 medical exam was a standard fee. Last I checked it cost between 110k - 130k

Medical is a standard fee but it may or may not not include vaccinations

YMMV

I recommend that if you are considering the "shop around" process for vaccinations prior to your medical exam that you do price comparisons but they must be against what your medical facilities is offering. So, the first call is to the medical exam facility to know what they are charging. If it is included in the fee then it's just less of a hassle to have them done with them then shopping around.

As I see it you have two choices. First is to get your vaccinations done there in your country as they are usually cheaper. Second is not to vaccinations but you will need them once you start your AOS submittal. At that time you will need to see a "civil surgeon" for them to either administer your vaccinations or to transpose them to the I-693 for your AOS package. A CS will charge you anywhere from $75 - $250 for this service. Either way you must have vaccinations to complete your AOS for the 2 year green card.

From the Form I-693 instructions (page 6-7):

"Even if a new medical examination is not required, you must still show proof that you complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original overseas medical examination report, you will have to have the Part 7. Vaccination Record completed by a designated civil surgeon. In this case, you must submit Parts 1., 2., 4., and 7. of Form I-693."

The medical exam you pay for does not cover shots and immunization those you'll have to pay if you need them.

In Thailand you'll have to pay for the shots if taken during the medical exam, guess it depends what country.
