Report of Marriage to the Philippines?

My Filipino wife and I got married about 17 days ago now and want to report our marriage to her country.  We have a quick question and are a little worried.

 

 When my wife signed the marriage certificate she signed it as her first name, last name, maiden name.  But on her passport she only signed it her first initial of her first name and her maiden name.  The wedding officiant told her she need to sign her whole name all the way out.

 

E.g.  Jane Doe Smith   Vs. J. Smith

 

Is this going to be an issue?  Also, the signature look different because of different pens and the way she signed.

 

She is worried this will be an issue to get the marriage reported and for her to get a new passport soon too.

I don't see that as being an issue, even if the signature is different.    With ROM those documents will be notarized thus verifying the signature is of the person named on the form.  :thumbs:  

 

 

 

And yes some documents require full signature.  ;)

For #6 I would put "single" for both of us right if we were never married before we got married correct?

For #8 what does it mean Naturalized? I am a US Citizen and she is a Filipino citizen so can I just leave that blank?  I'm just not sure what to put there.

For #12 and #13 I can just put any witness that was there or must it be someone official or something? Like can I put down my mom as a witness or should it be the pastor of the church?

For #14 I'm not sure what to put there

For #15 I'm not sure what to put here either.

For #15 I'm not sure what to put here either.  PASTOR'S TITLE

Not sure on that one.   Don't list her as a witness to the marriage on the ROM form would be my suggestion if you do.  ;)

Yes she is married, but the social security office wouldn't let her change her name to her married name until we have our EAD or AOS (green card). So it is still in her maiden name. Also, the whole ROM is filled out in the maiden name as that is what they ask for. So that is why we are confused about it....  Thanks.

Doesn't matter what SSA "won't do" ... sign everything in her married name!

 

Yes the ROM form is completed in her maiden name, but is signed in her married name.
