1. A question that is replied 'No' to and follows with "If yes, complete questions - to -", do you put down N/A or leave blank? 

 

2. I don't have middle names, do I put none or N/A?

 

3. For addresses with no province or postal code, do I put none or N/A?

 

4. If a name doesn't fit in the box, do I type out as much of the name as I can and then type the whole thing in additional information, leave blank or write see additional information?

 

5. If it asks for passport information or travel document, what are examples of a travel document? Like my non-immigrant visa?

Having to think about the tiniest details. Now thinking about it, additional information seems more like a section for added information as a whole so I will try to fit my names in either by hand or with a smaller font :)

If you were granted entry based on a visa in your passport, and received a stamp, you can leave 'travel document' blank.

 

The instructions say very clearly:

 

Passport and Travel Document Numbers.

If you used a passport or travel document to travel to the United States, enter either the passport or travel document information in the appropriate space on the application, even if the passport or travel document is currently expired.
