On Part 14 you have to type your name in again at the top (1a-1c). However, I'm using the additional information section for my name.

My last name doesn't fit in the space. Since I will be already adding information about my last name (type out the whole thing), do I need to do it also for 1a. on this part (14)?

Also, since it doesn't fit, do I leave it blank or fill out what I can in sections above but still include the last name with all names in the additional section?

For example:

Part 1, 1a. Last Name: Jones Smith

Part 14, 3d. Last Name: Jones Smith Lopez

I might need to clarify, this is hard for me to express without just pointing to all the sections!
