my question is, can i marry my canadian citizen fiance, in a different state in the USA or in her province in canada, then file for the paperwork in my home state?  Currently she is 5 months pregnant, and we are getting scared that any flights may effect the baby's health and welfare

Yes. For a CR-1 the marriage must be legal in the place where it happened. That's all. No requirements regarding location. You can get married anywhere. 

Timeline in brief:

Married: September 27, 2014

I-130 filed: February 5, 2016

NOA1: February 8, 2016 Nebraska

NOA2: July 21, 2016

Interview: December 6, 2016 London

POE: December 19, 2016 Las Vegas

N-400 filed: September 30, 2019

Interview: March 22, 2021 Seattle

Oath: March 22, 2021 COVID-style same-day oath

Of course you can.  My husband and I married in BC where I grew up vs marrying in Colorado.  It doesn't matter WHERE you marry as long as it is a legal marriage.

You can do the legal ceremony now and follow up with a wedding at your convenience, where and whenever it suits you. I130's out of Chicago are running about 7 months right now - the sooner you file, the sooner she and the baby can join you. If you do decide to marry in the US or Canada - it's fine either way. The only caveat is it can be tricky to enter the US for a wedding - there is the risk of being turned away because there's more likely to be an overstay/AOS filed.

Also the pregnancy may be high risk.  But in general, beyond becoming ill, or going into labour there is little risk.

Yes, maybe at 8 or 9 months.

However, that precaution applies to being anywhere that potentially can keep a pregnant woman or anyone with a health issue, away from medical care.

The point is that flying does not affect pregnancy.
