Hello.

Everywhere I read it says we have to include my wife's I-94 form, but we never filled it out and she didn't have one. Is this gonna be a problem?

Also I read that I can't send documents to Chicago lockbox in a binder like I did When we filed F-129. How do I send it in.

The I94 is done electronically when she entered the US.  Use the link Troy P provided to get her I94.  As to binders, in general they are unnecessary as USCIS will just remove anything in a binder to put in a file.

The binder is not really appreciated on the receiving end. There is a poor guy/girl who has to disassemble them and put the paperwork in the order they want/need it. I suggest paperclips (staples being a pain as well).

We just used lettered tabs that corresponded to a lettered table of contents in the cover letter. We did this each step of the way with no problems.
