Seems fairly clear an I212 is needed, not enough info to comment on I601.

I was entering Canada via Buffalo in 2014

My passport was scanned at the US border on the American side by a US border patrol officer before I entered Canada

yes they do have exit controls

you may have been a part of a pilot project.  this is not normal

If you know there will be a ban and you'll need a waiver, don't do the medical as it will expire and you'll have to do another one.
