I haven't put my middle name on my DS-160 since it wasn't asked, and I've read somewhere that I need to put it there if it was on the passport. Should I redo the ds-160?

Middle name is part of GIVEN NAME

But that middle name is my mother's maiden name. Shouldn't I put it in the surname?

Nope. Only your last name is your surname. Your maiden name is entered with your first name.

Redo the DS-160 with the proper naming nomenclature; First name, Middle name and Last name (surname). Print off a confirmation page with barcode and present only the new one at the interview.

Hi Greenbaum,

Just want to ask. For my DS160 I only put my surname in the last name and my given name in first name. I didnt include my middle name because that one was included in the section for middle name. Do I also have to redo my DS160? Hope you can help! Thank you!

You can redo it to make it correct. Just follow the directions in your quote that I instructed @throwaway

You do not need to complete a new DS-160. When at the interview ask the CO to update your DS-160 to include your middle name with your give name. Not a big deal.

You can redo it to make it correct. Just follow the directions in your quote that I instructed @throwaway

Not necessary, easier to ask the CO to make the update. ;)

Hi just want to follow up on this. Its my interview on monday. And honestly i havent included my maiden name in my ds160 form as i really dont know where to put it. Will there be a problem durinf my interview esp with regards on approving my visa? Please im very anxious and worried.

Do you mean middle name?  If so, your middle name should be part of your given name on your DS-160.

Hi @Ate yes this is my problem I havent included my middle name in mya last name. Will there be a problem during my interview? Or is it okay to tell that CO about this?

Tell the CO to add your middle name to your DS-160.  Don't worry.  I'm sure you're not the first visa applicant who has asked them to do this kind of update.

@Ate thank you. I think I’ll have to risk this mistake and you’re right maybe there were applicants who commited the same mistake as mine. Praying to get my visa approved. Thank you again

You're welcome.  Good luck with your interview!
