Hello, I received an RFE today regarding my I-765 and I-131 that reads:

"Service regards and information provided on your form has conflict regarding your full legal name. If the name you provided on your form is a married name, please include a copy of your marriage certificate. Please provide a sworn affidavit indicating your full legal name. Please include a list of all aliases. Pleade include your maiden name. For all names provided, please list them in the following format: LAST, First. Middle"

My question is, what exactly do I all need to provide with this? I did apply with my married name, but most of my documents are still with my maiden name because I did not get a SSN in time before my I-94 expired to change anything. So I am kind of in limbo. When I applied, everything I read said I could apply with my married name. so i am a little surprised with this RFE, but i am wanting to do all i can to reply as quick as possible and as accurate as possible. What is all aliases? And how do I include a list of this? What is the best ways to prove my married name and that I go by it. Since I still only have a Canadian Licenese with my maiden name and a Canadian passport with my maiden name. I have car insurance/licensed with my married name, and mail with my married name. Would that help to send it? I really want to work as soon as possible so I want to send in all I can to be approved. Thank you!

This may be a silly question, but did you attach a copy of your marriage certificate in the I-765 and I-131 packets? I sent 3 copies of my marriage certificate in my AOS packet, one for each set of forms (AOS, AP, EAD).

Not a silly question. I just looked at my personal copy of my AOS packet and i absent mindedly forgot a marriage certificate with the I-765 and I-131,had someone had asked me before looking back I would have confidently said I had, oops. My husband and i are going to go to our county clerk office tomorrow to get two more certified copies of the marriage certificate. Is that pretty much all they need? And a certified copy would probably be better than a photo copy, right?

Thanks!

In all honesty a photocopy will suffice in these matters. Don't waste your time getting certified copies. Be sure to send the RFE correspondence back with your submission.
