Question about part 3 of I-90

Hi folks,

I have another question and I hope that's the right forum.

I need to replace my greencard and file I-90. But I'm a little confused about Part 3. If you guys could help me out :)

Part 3
3.1
Location where you applied for an immigrant visa or adjustment of status: Do I enter the immigration office where I got the IR1 granted or the CR1?
3.2
Location where your immigrant visa was issued or USCIS office where you were granted adjustment of status: To me it seems that this question is the same as 3.1. What would I enter here?
3.a
Destination in the United States at time of admission:
Was that the address I gave the LAX officer? Thing is, I can't remember what I gave him. I went to Utah. Do I put that in?
3.a1 Port-of-Entry where admitted to the United States: City or Town and State: I flew into LAX. So I would put Los Angeles, CA?
Many Thanks!

Did you have both an IR1 AND a CR1???
3.2
Location where your immigrant visa was issued or USCIS office where you were granted adjustment of status: To me it seems that this question is the same as 3.1. What would I enter here? Some people's cases were transferred to a different location so the location at which they applied would be different from that from which their visa/AOS was granted. If your case was not transferred, the answers to 3.1 and 3.2 would be the same for you.
3.a
Destination in the United States at time of admission:
Was that the address I gave the LAX officer? Yes. Thing is, I can't remember what I gave him. I went to Utah. Do I put that in? If that is all you remember, then, yes.
3.a1 Port-of-Entry where admitted to the United States: City or Town and State: I flew into LAX. So I would put Los Angeles, CA? Yes as it is the City and State where you entered.
Many Thanks!

I'm still confused about what to put at 3.1 and 3.2. Do I put my first visa (Frankfurt) or the one when I got the conditions removed (Salt Lake City)? Help is appreciated :)
