If you look at Page 12, Part 11, you can see clearly that there are now extra spaces for adding extra details and attachments to questions as needed. It also tells you that you can also go ahead and use an extra sheet of paper if there is not enough room on the form, and that you should include your A#, the form number, page number and question number you are adding the statement/attachment for at the top of the page. Read the instructions written right there under Part 11 Additional Information.

And yes, it would be page 6, part 6 and item 20.

This is a new form edition that I have never used. I am simply going by what the instructions of the form state. I am assuming they added this part so adjudicators don't need to go through or miss extra, added supplements. If you don't want to use Part 11, then just add the statement as an extra sheet attachment.
