Sorry meant CFO Commission of Filipino Overseas. Below is from the DFA website for change of name to passport

For spouse of foreign national:

Marriage Contract (MC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or Certified True Copy (CTC) of MC issued by the Local Civil Registrar (LCR) and duly authenticated by NSO. Transcribed Marriage Contract from the LCR is required when entries in NSO Marriage Contract are blurred or unreadable.

Report of Marriage duly authenticated by NSO if married abroad

Original and photocopy of Commission of Filipino Overseas (CFO) Guidance and Counseling Certificate of Attendance (required for first time applicants or renewal of passport to be used for the first time the surname of the husband)

yes once the I-130 petition is approve at the USCIS it will get sent to the nvc. You will send Two packets together once it is there.The iv packet for the benneficary it should include a copy of the nso marrige certificate and copy of nso birth certificate. Nbi clearance and police records from any other countries live in. The second packet will be the Aos packet it will include the finicial record of the petitioner

Only send copies to the USCIS and the NVC

keep originals for interview

Thank you so much for your reply, if my husband will send the I-130 he will also includes the evidence of our marriage, right? We just got married last aug 1,2015 and the NSO of our marriage cert will be relase by sept, am I going to send it to him the original copy? But he will only make copy of it once he send it together with the I-130? I'm totally confused...thank u

Do I need to do it now at CFO? Because I just renewed my passport 2yrs ago..my drivers license will expired nxtyr so that's the time I will change it.im going to start to change my status on company IDs and some legal documents

yes he will need two copies one for USCIS And one for Nvc

Just make a couple of copies and send to him or he can request a copy at www.ecensus.com.ph

Do not send the original copy you will get as you will need that for your interview later

Your husband will send it so he needs Two
