So I got my package back in the mail with this notice of action letter in it http://i.imgur.com/u1zfODZ.jpg

I sent a copy of my signed I-751 form. They only want a copy of the form, correct? not the supporting docs?

The photocopied form came back with a stamp and some writing on it. what does it mean? http://i.imgur.com/NlEbKfX.jpg

Also, when I send it tomorrow do I include the notice of action letter at the front or some sort explanation?

It clearly mentions they want the form with your original signature using a pen (not a photocopied one) and you are still talking about they want a copy of the form. DO NOT send them a copy of the form, send the form with your original signature.

It clearly mentions you resubmit the whole application with all supporting documents and again you are talking about they don't want the supporting docs. SEND all supporting documents.

Disregard the stamp and whatever was written, it was for their purpose.

It again clearly mentions to resubmit the application with the notice of action letter. So yes, include the notice of action letter. No explanation necessary.
