We are in the process of filing for AOS and have a question about the I-693 form. We have a sealed envelope from the medical examiner (from October of last year) but we aren't sure if that's all we need. It says that if you had the medical exam within the last year you do no have to have a new exam, but we aren't sure what is in this envelope - whether it is what is all that's needed or not. It is sealed and we do not want to open it in case it needs to remain sealed. My husband gave another sealed envelope in the packet when he arrived.

Also, even if this IS all we need for the medical exam part of the AOS, do we still fill out the I-693? Or is its use only to request a new exam?

I hope these questions make sense. Thank you for any response!

If your DS-3025 is check marked complete then you don't need it transcribed.

Our DS-3025 copy given to us was not sealed.

AOS send : May 12, 2015, received May 14, 2015 USPS priority

Biometrics : June 1, 2015 letter received for appointment June 8, 2015, successful walk-in June 1, 2015

RFE : June 12, 2015 for income not meeting guideline. Income does ( ! ) exceed guideline.

RFE response : June 26, 2015 returned with a boat load full of financial evidence.

UPDATE: July 5, 2015 updated on all 3 cases, RFE received June 30, 2015.

Service request : Aug 12, 2015, letter received that it will be processed within 90 days from receipt of RFE.

UPDATE: Aug 24, 2015, EAD card being produced/ordered. ( 102 days from AOS receipt day and 55 days from RFE response received.) Thank you Jesus !

Emails : Aug 24, 2015, EAD approved, EAD card ordered.

I-797 EAD/AP approval notice received : Aug 27, 2015

EAD/AP combo card mailed : Aug 27, 2015, EAD/AP combo card received: Aug 31, 2015

Renewal application send for EAD/AP : May 31,2016 (AOS pending over 1 year). Received June 2, 2016,Notice date June7, 2016, emails,texts, NOA1 hard copy

Renewal EAD in production notification text and online, expedite successful 4 days after RFE request response was faxed, Aug25mailed,Aug29received.

Sept. 21 Interview, 2 hour interview, we were separated and asked about 50 questions each for an hour each. IO was firm but professional, some smiles.
Several service requests made, contacted Senator and Ombudsman. Background checks still pending.
July 21, 2017 HOME VISIT.  Went well. Topic thread in AOS forum.
Waiting to skip ROC and get 10 yr GC due to over 2 year while pending AOS
AOS APPROVED Oct. 4, 2017 * Green card in hand Oct 13, 2017 !!!!!

First K1 denied after 16 month of AP. Refiled. We are a couple since 2009. Not a sprint but a matter of endurance.

Hi I did my medical 2 weeks ago and receive a seal envelope and was told not to open it too. I advice you don't . The I-693 from should already be in the seal envelope, I saw when the nurse put in mines and seal it. Hope that help.

My husband never received a DS-3025. He has his "yellow card" the immunzation record and a sealed packet. But the other sealed packet was taken from him and he never received a DS-3025. Does this mean we have to have him examined again? We are very short on time and needing to file asap.

My husband just said that the medical records, etc they gave him was in an envelope. They took that from him at the interview. We are worried that in this sealed packet is only xrays.

Is there a way to get a copy of the information he gave them at the interview?

During his K-1 visa interview. They took the sealed envelope of the medical records that was given to him at the hospital when he had his exam (they also gave him the other packet we have now. I am strongly thinking that it is just xrays.)

Yeah we can see through the front and it definitely says "x-ray" on the first sheet. We just weren't sure if that was the only thing in there.

I'm not sure how to advice you on that, but my opinion is once they took in from him that's should already be in his file. If not when you make your AOS application the will request it before the interview.

Ok! I just read further into that post that you linked to me Ebunoluwa, and it actually has a mention if we do not have a DS-3025! Doh! Should have read it completely first!

NOW, if we do not need to submit the copy of a DS-3025 because they have the original, do we still need to complete and send in an I-693?

SHEWS this is all so confusing! I really appreciate this site and all of the helpful posts! You guys are the only thing keeping us sane at this point! :no:

At the medical exam the beneficiary is given a sealed packet to give to the CO.

You take the sealed package and hand it over at POE.

This packet then will meet up automatically with your AOS, it includes your medical and original DS-3025.

No one cares about the x-ray that's yours to keep.

IF you were given or requested a copy of the DS-3025 then and only then would you know if the vaccs are complete or not

and could then decide if you need to see a civil surgeon to transcribe any further vaccs to the I-693. The I-693 is for a CS to fill out not you.

They would RFE you during AOS if your vaccs record would be incomplete.

The medical is good for one year from date of exam to filing date for AOS.

I suspect what you have in your sealed package now at home is the x-ray and perhaps a copy of the DS-3025 which is yours to keep so you can

check to see if it is marked complete or checked eligible for a waiver.

You never open a sealed medical package before or after the interview that is correct but he has already gone through POE and is already here

Yes, they took his packet at POE. So then there is nothing more we need to fill out as far as AOS packet goes, right?
