Are you getting ready to file the petition? If so, you only complete one I-129F form and make sure you list the child on the form. You only fill out a separate form when you get to the stage of applying for the actual visa (it's the DS160). That step is only undertaken once your petition has been approved by USCIS.

Thank you for the fast update of information . Yes I am getting ready to file and want to make sure I don't messed up . I have 4 kids all underage , in the form only 3 kids are stated , do I need to write in a separate sheet like an Addendum?

Yes, if you run out of space, add a separate sheet. Just make sure you remember to include it in the pack when you submit it and mark the sheet with the question number and make a note that it is 'continued' due to lack of space on the form. I'd also put my name and date of birth or some personal reference on the extra sheet so that if it comes apart from the pack, they can match it up. You can't use staples on anything in this pack so get some good paperclips. Good luck :)

Or just print that single page again. And don't write any of the children's name on the form so it's not different names in two different sections.

Make it easier for the Co and write see attachment, and write the children's names on there so they are are all in the same place.

Are you just starting the I-129F petition process? You need to list the child on that form, Part 2, that is is until embassy time. Then at the embassy phase of the process you will be doing two visa applications.

Yes for the 4th child do a separate page referencing to Part 2.
