I got my B1B2 has annotation "Medical Clerkship in Texas in June and July." Will there be a problem in the port of entry at the airport if I traveled in April and my first ARRIVAL state is Washington state ? I am from the Middle East.

No

Arguably clerkship is outside the normal parameters of a B.

It is okay. As it stated in Foreign Department of state Manual ) Volume 9) that medical student aiming at clerkship applies to B class.

I am just wondering what is the purpose of that annotation on my Visa ? Am I should be assigned to Texas only ?

I know it is OK, you know it is OK but your average immigration officer may not.

It adds clarity.

So what is the importance from that annotation on my visa ?

The annotation was put there based on what was written in your DS-160 PLUS the conversation you had with a Vice-Consul at a US Embassy during the interview.

Do you remember any of that conversation?

If you are coming to Houston for medical training, it's good that this annotation is there, removes some silliness with any ICE folk who may want to study your visa in Houston.

Sometimes my language usage seems confusing - please feel free to 'read it twice', just in case !
