How to fill up Report of Marriage for Philippine General Consulate in LA

Hi everybody.I want to ask help for those who knows how to fill up a Report of Marriage form that will be submitted to Phil. General Consulate in LA.I know once you get married here in US you have to report it in Phil.Consulate.I'm confuse if I have to fill up the form like what was written in my passport or I have to use my husband's last name.I don't know if I have to use US or Phil. as my residence.In addition to that I change my signature as I change my status into married.I hope for response.thank you very much
