Hi everyone

we are just going through the aos paperwork but i have got a bit stuck with the i-693 form and what to do ??

do i just fill this out and attach my medical record from the UK medical i had ??

my ds-3025 is filled out i have with me here do i just copy this and send it in ?? plus the CD ?

what is the i_693 for then ?

If your DS-3025 is filled out correctly with every needed shot, then send a photocopy. But if you still need a shot or got one after the medical at your GP, then you need a new form (I-693) filled out by a civil surgeon.

Chances are you don't need the I693.

The 3025 you have is a copy and the original is in the pack you handed over at POE. I just sent a copy of 3025, no I693.
