CR1 interview approved but still no visa. Was not issued a 221g

My husband and I had our interview on Nov 21, 2013 and were told we were approved. The officer told us that we would be contacted in 7-10 days. After two weeks I called to inquire and was told they would call the next day. They did not. I called a few days later and was told our case was still pending. Of course when I asked to define "pending" i received scripted answers. She did say that she hoped that we would have an answer by the 23rd of Dec. I wanted to login to the CEAC sit to check the progress, but the case number given to us by the Embassy here in Athens does not work.

questions: If we were told we were approved and they kept his passport and was not issued a 221g should there be a concern on our part.. and what is the usually cause of the delay considering they told us initially that they could contact us in 7-10 days

secondly. How can you access the CEAC website if the case number given by the embassy does not work. I emailed the embassy to ask this question and their response was " As we discussed in our phone conversation, your husbands case is still pending". But they would not answer my question on how to access the website.

It's possible that it is as simple as the visa printing machine running out of ink.

You could try running DOS and see what the case status is in their system.
