DS157 - in the instructions it says to fill in the DS158

Hi folks - thanks in advance for any help.

I'm the USC. My Filipina fiancee completed the DS157, DS230 and DS230 and i'm double checking them, so i read the instructions online for the DS157 and it says

VISA APPLICATION SUPPLEMENTS PROCEDURES
(DS-157 for MALES and DS-158 for ALL)

The instructions are here http://www.ac-see.org/documents/SYLPVisaInstructionsEN.pdf and it's immediately after the 41 questions.

I have not heard it mentioned anywhere else before so i was wondering if anyone else knew of the DS158? Maybe i'll get her to complete that too, it can't hurt i guess.

Look here for the instructions from the US Embassy in Manila: http://photos.state.gov/libraries/manila/19452/public/Revised%20K1%20Instruction_rtf2_rtf2_001.pdf

She only needs to fill out DS 156, 156K, 157 and 230 (part 1). Other required documents are also listed there.
