if you paid online at NVC,

you're supposed to download the coversheets (there are 2 - one for AOS, another for IV)

and follow the instructions in the coversheets, from there.

Ya know, you can find the answer to your question with the advanced search tool, when using a PC? Ditch the handphone, come back later on a PC, and try again.

Thank you, Darnell. I paid the $230 for DS-230and $88 for AOS. I submitted the requirements for AOS however I missed the DS-230 form. So I need to do is fill up the DS-230 form for my mom and me? Does the DS-230 have any additional requirements? I know I sound so stupid but this while thing gave me too much confusion esp. now that they are doing everything electronically. Thank you very much for your time!

Your mom will fill up the DS-230 form.

Darnell, She is in the Philippines! What are civil documents? The way they design this processing right now is very confusing. Not like before that they send you some steps to follow in the snail mail...Thank you for all your help!

No, it needs her original signature. Only the Part I (page 2) needs her original signature & leave the Part II (which is page 4) unsigned. The Part II will be signed during the interview.

Your mom has to send you her civil documents like (BC,NBI clearance), 2x2 visa pics, photocopy of her Bio-page of her passport. You will be the one to send the DS-230 packet to NVC.
