A bit of an update, I've been to my GP a couple of times and he said after reviewing the paperwork of my dad's heart attack, looking at my hospital results etc, it is highly unlikely I have the same condition as him and he will speak to the cardiologist to confirm, but he is very doubtful he will want to do anything. So to make sure I do not declare anything in my paperwork as no diagnosis has been made and I don't have it.

Also about the self-harm thing, he said he doubts they will want to refer me to a psychologist as it is so long ago and I don't have a big history with it, but as long as I pay as it's outside of the NHS cover, he will write me a letter stating I am no threat to myself or anyone else.

Pay for the letter. It's your golden ticket. Sounds like things are looking up.
