If my fiancee does not have a middle name, is it still necessary to put down N/A for the middle name box?

Describe the circumstances under which you met: What exactly are they looking for? Does it mean how and where we first met?

Our engagement was arranged through our family and the first time we met was through video chat and we agreed for the engagement. First time i met my fiancee personally was on our engagement. So, what would be a best way to describe the circumstances?

Another Question: Question (9) Dates marriage ended. I got the divorce decree on Aug 6th, 2012 but there is a six month waiting period before i can re-marry. So, the date marriage ended should be Aug 6th 2012 or Feb 6th, 2012 (which is six month from Aug 6th)?

Question 18 wants you to describe how you met in person within the last 2 years. Nothing more.
