So my fiance spent $95.00+ to UPS his I-134 form with original signature to me, but to my despair, has put his incomplete name in the Name field. THE VERY FIRST ITEM ON THE FREAKING FORM. I swear to God, sometimes, I can just strangle the man. I even reviewed his form and told him to correct that field specifically before he sent it out to me! Anyway, I don't want to call him and launch into full harpy mode (it's 5:34 a.m. where he is).

Can I cleanly strike out his name with a black pen and write his full name in that space? Example:

I-134

I, Rafael McCareless Rafael Joseph McCareless residing at 1** W. *th St., Apt. *

(Name)

Please help.

Look up the form I messaged you as it appears I can't post the link here. On the sample form they write the First and last name in #1 and you put the whole name with middle name in #3 . I think he has just followed these instructions so you don't have to amend anything as long as his middle name appears in #3

I don't think you have to worry :)

Disregard/remove my first comment. Too early in the morning .... #1 My husband did the same (he put one line through in black ink.. initialized and rewrote the name with the middle name next to it and when I phoned the USCIS they said it was ok as long as it looked clear.

Anyway. It shouldn't be a HUGE deal if he didn't write his middle name on the form. Don't strike it out just yet. If I were you, I would leave it as it is and to ease my mind, I would email the consulate presenting the situation, the same way you did it here. So, if they reply saying... "Yeah! It's fine! Don't kill the poor man..." you can print it out and take it with you to the interview (in the event someone tries to tell you otherwise). Also, because if they say "No, we need the entire name"... you can show it to Mr. McCareless and ask him to make a new one.

Makes sense?

Thanks guys!

I did call the Manila US Embassy (like Celeste suggested) but the lady I talked to told me that, basically, I can't edit anything on the form once it's signed, only HE could (like MR193's husband did). She wasn't clear whether they would accept the form without the full name, though. I am calm and level-headed now - I don't think it should cause a serious problem since I reviewed some of his other official papers and some of them had only his name (not full) in them. If they require us to resubmit that document during my interview, that's when he'll drop another freaking $95.00 to send me an updated one. (It's a good thing we're in no hurry. I'm just thinking it'll be such a waste because we could spend that money for wedding favors!!!)

I wouldn't worry about it too much at this stage. It's not legally binding.

My idea though is thus, he didn't sign on the first page, so get him to email the form he completed to you, fix the name and print just the first page and attach that. Easy.

Have him fill out a new identical(except for the full name) page 1 and use Vanessa&Tony's idea. I'd simply X out the original first page. No harm done in that, and you still have the original signature along with a form filled out completely by your fiancee

I in some way hope he doesn't see your topic because he might put McCareless as the last name. On the other hand I hope he does.. Because not many knows that they have a fiancee with a sense of humour despite being angry. : )

I thought of that too but he printed it back-to-back. :-(

Then don't worry about it. Like I said before, it's not legally binding anyway, and it doesn't actually specify first, middle, last so it might still be fine.
