downloading Cover Letter k-1

I've tried downloading the format for downloading this sample form, but it will not open...can anyone send me a sample of it that we have here please?

Also I try to view K-1 guidelines from here, but I keep getting error message " not allowed to view this forum".

Anyone know what gives?
