can somebody pls clarify this things to me..i have a notarized parents consent..somebody posted here that the consent should be hand written and someone also said that it has a corret format? (sorry but i am really lost im gonna have my CFO this monday 14th of may)

Looks like you have two Philippines government appointed forms under the Family Code. Depending on your age, I would use the appropriate form. I wouldn't go by hearsay if the government has actually issued these forms to use.
