Looks right. Just to confirm: Send a copy of both front and back of your greencard.

To receive an Email and/or text message when USCIS receives your application, I would suggest that you include the following with your application as well:

Form G-1145, E-Notification of Application/Petition Acceptance which can be found here:

No need to send in the tax return transcripts at this point. Take them with you when you go to the interview. I took transcripts for the last 5 years.

It is neither 1040 nor W2. You need to order an official tax return transcript from the IRS. It is free to order. You can either order online here: http://www.irs.gov/individuals/article/0,,id=232168,00.html

or you will need to fill out and send Form 4506-T found here: http://www.irs.gov/pub/irs-pdf/f4506t.pdf

If you use Form 4506-T, in section 9 of this form, write in 12/31/2007, 12/31/2008, 12/31/2009, and 12/31/2010. That is four years. Since you have not filed for 2011, yet, you can order that after April 15.

You can go to the local IRS office and get copies of your tax returns too. Just take your id and they will print it out for you for free.

You can also order them online at http://www.irs.gov/individuals/article/0,,id=232168,00.html
