Hi guys... time for me to fill up DS form . im just confused for other question here how to fill up

No. 8 Other Surnames Used (Maiden, Religious, Professional, Aliases)? do i need to put my Middle name here or to

No. 9 Other First and Middle Names Used)? this is the one i need to put mu Middle Name?

No. 33. Do you intent to study in usa? Im not yet sure if im going to study there but my fiance want me to study there. what will be my answer here?

please guy help me.. on aug 15 is my interview in USemabssy Manila..

8. No do not put your middle name here. Other Surnames used means other last names you have used. Like in a prior marriage, or any other LAST name you have used that is not on your passport. Otherwise put NONE. Make sure in #6 that you have your last name or names as it is on your passport. And #7 put your 1st and middle names.

9. Also put NONE unless you have any other middle names not on your passport.

33. Put NO, because you don't have a school you intend on attending, but it doesnt really matter.
