This is what i wrote and got back from the US embassy in Manila thru facebook:

Me:

Hello, My name Is Jenessa Omadlao with case number MNL2011625113. My concerned is that the DS-156 form could be accepted if it filled out out by hand written? I have a problem filling it out, it won't allow me to continue when i answered in question no: 14(National identification Number) was not applicable and question No:15 in my home address has no apartment or house number coz i lived in a sitio only not in subdivision. The system would just said INCORRECT and it won't allow me to continue. Are the Street address line 1 and Street address line 2 can be the same Address? And the DS-156 was expired on May 31,2011,does that date still accepted? or is there DS-156 form, updated?..please write me back. Thanks and God bless!

"U.S. Embassy, Manila Philippines :

Hello Jenessa. Unfortunately, DS-156 forms are no longer accepted. Please try filling out a new DS-160 form and save each page regularly on your hard drive for reference. If you need additional assistance, you can e-mail ConsManilaNIV@state.gov. Thanks!"

Why the heck they replied me that?

and

I do have the question before? But when I thoroughly read about this, K visas are not included, we should fill up 156... Anyway i would suggest if ever you will really have some confusions, try to print both, and bring it with you during your interview, which ever they ask at least you have it.. I might do the same thing to be safe on my interview date.

try writting NONE instead of just NO..otherwise leave it blank and use a pen to write none. dont leave any space unanswered.

Or Use N/A

I think the previous post was correct in that the consulate assumed you were applying for a visitor visa. You can fill out the form by hand just make sure it is clearly filled in. I wouldn't go to the interview without the form.
