Form confusion

When filling out the package of forms , I-130, I-485 , I765 do i need to send 4 copies of each form with the rest of the documentation and keep a copy for ourselves .

You send just one of each form, and keep copies of everything for yourself.

I believe the only form that is required in "quadruplicate" is the medical I-693. Which is filled out by the civil surgeon and placed in a sealed envelope anyway.

So if my husband cannot find his original vaccinations he received in Russia, then we are going to have to do the entire I-693 over again? What about the supplemental form- where do we get that from?

You go to a civil surgeon in your area and have them do the supplemental. I have heard to call around to a few places because the rates are different for every surgeon.

yES SEND ONE COPY THEN KEEP ORIGINALS TRY CALLING DIFFERENT SURGEONS IN YOUR PLACE SOME ARE EXPENSIVE AND THEY WANT TO DO THE COMPLETE MEDICAL SO DNT BE FOOLED.
