Pkg 3 to Montreal - All forms?

Have a look at the consulate's official website at http://montreal.usconsulate.gov/content/content.asp?section=visas&document=index

For all applicants (including fiancé(e):

DS-230 part I and part II – one copy per each applicant, including children

Checklist of documents to gather for the interview – one copy per family is sufficient.

2 color photographs (Photograph Requirements)

Photocopy of the biographic data page of the applicant’s passport

For fiancé(e) visa applicants only, please complete additional documents as follows:

DS-156 – 2 copies https://evisaforms.state.gov/ds156.asp

DS-156K – 1 copy http://www.state.gov/documents/organization/82063.pdf

Form DS-160 – one copy (confirmation page only), available online at https://ceac.state.gov/genniv/

You can send in your package as soon as DOS tells you the consulate mailed your Packet 3. You don't need to wait for the physical copy but it has to be logged as mailed by Montreal.

The Montreal consulate page here on VJ needs some serious updating. There is so much wrong information on it.

You can send the completed forms without receiving the physical Packet 3 letter from Montreal if DOS has confirmed that it's been sent to you.
