3) question is confusing

YMMV

2x2 is US passport size.

The alien registration number is above the beneficiaries name on the K-1 NOA2

If you check your NOA2, above the beneficiary's name there is a number with an A in front, such as A 123-456-789. That's the Alien registration number.

what's the civil registry?

I swear i can't find anything in the noa but i saw it in the I-94 and my visa.

thank you so much.

What i meant was the civil surgeon.

do you think how much would it cause to complete the vaccination and all stuff?

my DS-3025 says it's incomplete.

thanks for the help.

You're not looking properly then. On the NOA2 (which is the notice that says "petition approved") there is the beneficiaries name and DIRECTLY above it is a number starting with A. Do NOT go by the number on your visa or I-94. It's not uncommon for there to by typos.

A vaccination transcription shouldn't cost more than $50 plus the costs of whatever shots you're missing. You only need to fill in part 1, Part 2 (vaccination table ONLY) and Part 5.
