I'm filling out my I-134 and want to make sure I'm getting it right.

Line 7: My life insurance agent told me that since I have term not a whole life policy, I do not have a "cash surrender value" even though I have a sum in the previous line

Line 10: I'm going to put my fiancee's name, but what date goes there? The day I have it notarized?

Line 11: The consensus seems to be that since its a permanent visa, I should check "do not intend" and put in the line "N/A (K-1 visa process for permanent resident"

Using the example forms are VERY helpfull, you can find them under the example form button at the top of the VJ page.

The I-134 is not notarized.

Edit to add. You should not be filling this out currently unless you have an NOA2. This form should be filled out as close as possible to the interview time and sent to the beneficiary.

Your I-129f was approved in 5 days from your NOA1 date.

Your interview took 67 days from your I-129F NOA1 date.

AOS was approved in 2 months and 8 days without interview.

ROC was approved in 3 months and 2 days without interview.

Since I-134 is NOT filed with I-129F when filing petition and is used only at the US consulate or embassy, this topic is being moved to Embassy/Consulate forum, there you will find I-134 questions asked many times.

7) What is your income?? if sufficient then don't use assets.

10) I-134 no longer is a form that gets notarized, that changed a couple years ago.

Treat the I-134 as if it is like an I-864"Light" so use the same financial evidence as you will later for the I-864 when adjusting status, (I Used Past 3 years simple IRS transcripts, or copies of IRS returns, Copies of past 6 months pay stubs, letter from employer..)

We wrote for line 11

Yes I do intend and in space provided my husband wrote intending to marry and thus share our marital wealth.

Yes line 10 your fiancees name

134 not notarised .

Divorced !st November 2012.

Married only 2 years 1 month

Sorry, I should clarify what I was asking

On Line 10, the form asks for persons whom I have previously submitted a visa petition for. Now that I read it again I realize it is asking. So I assume I should put my fiancee's name and maybe the NOA1 date? I've looked at the example form and I just want to make sure I get the date they are asking for correct.

And on Line 11, from what I've read on previous posts, some people check "intend" and others "do not intend" and some even do neither, just as long as an explanation is in the blank. Does anyone know what the Manila Embassy prefers?

I did not check that box, that section has to do with other Non-Immigrant visa types such as Work, or Student visa in those cases a sponsor can specify how much support. 11 does not apply to K-Visas.
