Hey everyone! I'm currently a k1 visa holder and just wanted to clear something up. I need 2 vaccinations by the time I apply for aos and was wondering the following:

can I get my vaccinations done anywhere? Then take the documents and forms to a civil surgeon to sign?

And I know I fill in just part 1 of the form, but what else needs to be completed? Does the civil doctor just sign them?

Any help would be amazing, as I'm hoping to apply for aos pretty soon!!!

The CS transcribes the vaccinations onto the sheet then signs it and stamps it and seals it. You need to bring your vaccination sheet for him to see to sign them off. Call around to different ones in your area. They are very hard to find one that will sign off for you without a new full medical. They charge crazy prices too because they can.

I dont really know if you can get it done " anywhere " because US medical care is strange.. The CS will give you the shots if you need any extras, unless you want to find some other doctor to do it but i cant immagin that it would be cheap.

Yes you can get the shots anywhere in the US, as long as you take the records with the vaccination checklist to the CS to sign Part 2 (vaccination table only) of the I-693 you're sweet.

When the CS transcribes the documentation they will return it to you in a sealed envelope. DO NOT open it...send with the rest of your AOS stuff.

Thanks so much guys, one final question!!...the A#, is this the one on my NOA2 or the one printed in my visa in my passport. The number is exactly the same except the number on the NOA2 has a 0 at the beginning of the long number, turning it from a 8 digit to a 9 digit number. any suggestions?

Yes that is your A# You can put the 0 infront of it or you can leave it off. USCIS will just add the 0 to it later if you leave it off tho lol..
