Hi to all Vj members,

I'm trying to fill-out some forms from the philippine consulate website and I'm having a hard time with one of their forms (report of birth).

Everytime I typed in my daughter's name on the 'first name' (name of child), her name will also be automatically on the 'full name' where my husband name should be and vice versa. Same thing happens on her maiden's name.

I can't also type any letter on the blank next to 'sex' and 'Am' 'Pm'.

Everything was filled out except those that i mentioned above. any suggestion?

Is it ok that the Affidavit for Delayed Registration and Ammendment of Passport handwritten?

Do it need to be notarize now or It should be done at the Philippine consulate office?

My wife and I had the same problem when sending the Report Of Marriage Form. We typed in the spaces that allowed it and the ones that gave us problems we just hand wrote it. We had no problems doing it this way. If you are mailing in these forms to the consulate then you will need to have it notarized before mailing them. I sent ours and had the Report Of Marriage sent back to us within 2 weeks.

We're going to Chicago this week and renew my passport and report the birth of our kids.

This are the forms I have:

1. e-passport application

2. application for ammendment of passport

3. Report of Marriage

4. affidavit for delayed registration (should this be notarize now or at the consulate?)

5. Report of Birth

I tried calling the consulate but no luck. they just keep me on hold.

any other info that I need to know before going there? Am I forgetting anything?

I looked at the form for the affidavit for delayed registration.It appears that it does need to be notarized. Probably would not hurt to have it notarized before going to the consulate. I do know with the report of marriage form if you are not making a personal appearance to the consulate then it needs to be notarized before mailing. It appears that you have included all the forms needed to bring with you.

Hi. Just a comment that you may want to think about and consider. Since the Philippines has just made the 2nd change to their passport format in only 3 years. As of this past summer, 2010, Newly issued Philippine passports are so called "smart passports" (aka "e-Passports". This new type of passport has replaced the previous "Machine Readable Passport" format that was only implemented less than 3 years ago. (a lot of turmoil for such a short lived format!)

Since I noticed that your list included both an e-Passport application and an Ammendment of Passport application, I assume that the e-Passport Application is for your child and that the Ammendment of Passport application is for you to change your maiden name to your married name. You may want to consider NOT filling that Ammendment Application, and instead, filing an e-Passport Application for you (which can have your name change incorporated) so that you will have an e-passport, not an old style "Machine Readable" or the even older Non Machine Readable passport.
