Hello friends,

I have a question regarding which name to use as the surname for my fiancée. On her passport/birth certificate her name is written as Jane Doe John-Smith. Where John is her father's surname and Smith her mother's maiden name. When it asks for my fiancée's family name, do I use John-Smith or just John?

I apologize if this question has been answered elsewhere...I'm a bit new to this informative site, but could not find the answer I needed in a brief search. So any help would be appreciated.

I'm pretty positive(without knowing exactly which form you're using) that you should fill in her name exactly as it appears on her passport. Most of the USCIS forms are like that.
