Civil Surgeon Fees in Seattle Area
hi - just a plea to anyone within the seattle area. im trying to get the civil surgeon to sign my I-693 form. I have had all my vaccinations. I needed one after my medical and have this now and have the DS-3025 form and my medical was only a few months ago so is still valid. However the surgeons i have called want to charge me anywhere between $85 and $155 just to complete the paperwork!
Does anyone know of a good place to get this done.. that just seems way too expensive and much more than the
