I just got my I-693 transcibed. It was pretty much smooth, no hassle. In and out in 5 minutes and the staff are really nice. Cost is $35 dollars for transcription. Recommend it to anyone in the Reno, Nevada area.

I wanted to drop a line for Immigration Medical Exams of the Northeast, run by Vivian Esparza, M. D. located near Burlington, Vermont. Convenient for people near the St. Albans USCIS. My wife came on a K-1 Visa and had a DS-3025 vaccination supplement that cited "Shots Completed", but I didn't want to risk the MMR #2 missing and not having an I-693, to be RFE'd in the long run. I paid $85 altogether for the vaccine and the transcription, and it was well worth the sacrifice. The vaccines are reasonably priced and are relatively far cheaper than a walk in immunization clinic.

He charged 170 dollars for adult(included testing for TB(ppd), Syphilis and HIV) and 110 dollars for kid.

On top of that, I had to pay 72 dollars for MMR and tetanus shot for my wife(she had no vaccination records). My kid had all vaccines up to date and we got a copy from her PCP.

All other doctors were asking for 200-375 dollars.

Dr. Torres seemed pretty knowledgeable about requirements related to immigration. Only thing is he works out of a fairly small office, and doesn't really take appointments. Hence I had to wait 2 hours for him to work through other people waiting before he got to us. Once he did, only took about 10 minutes for him to complete the transcription.

It was not cheap, but Dr. Hart spent much time to find out what I needed for my medical. Also, all his staff members were nice. I am happy with this service.

As "judi" mentioned above it was quick and painless... and funny :) Dr. Urda was telling jokes all the time I was there :)

Their staff is very knowledgable on the latest immigration updates. All you have to do is simply explain your individual case and they will help you right away.

All I had to do was a TB skin test ($45). This price included transcription for vaccines too.

Here's a miracle in PENNSYLVANIA, charged only $25 to transcribe immunization record to I-693.

I just thought I'd let you all know about my CS appointment yesterday. Russ and I went to Dr Noel Smith in New York City and had an appointment for 12pm but were about an hour late as we had to walk so far and didnt anticipate the distance as we thought it was more up town than planned. Anyway, all is done and sorted out, thankfully.

However, if anyone plans to go here, please have your wits about you. I was given the hard sell BIG TIME, from the moment I stepped in the door. The first words from the receptionist were '$250 for full examination only!'. I had to really defend myself and had all of my info from USCIS ready to show them that that wasn't required. When booking the appointment over the phone, they also tried it on then but eventually agreed to $50 for the transribe only. I told the receptionist this and eventually he backed down, took a copy of my passport and said 'they'll sort me out soon'. So I was called into Dr Smith's office a few minutes later and was given the same speil all over again about how I need to prove I don't have syphillis, how I'm going to need a blood test, a TB skin test and 2 more vaccinations (when I'd had them two weeks earlier!) Do these people think you are stupid? Anyway, to cut a long story short I stood my ground and left with the I-693 completed and sealed and I even got a copy for my own records. I'd recommend going there if you have to but be prepared to stand your ground. Its also not the nicest place to be hanging around on your own. But at least its another step completed.

I was missing the MMR jab for my K1 medical but it was waived.. I had a hard time finding a doctors here that would just do the MMR and sign of section 2 (?) of the I 693...

I found a civil surgeon in Allentown, PA, that gave me the shot and signed off the vaccinaton part of the I-693.

The shot cost $110 and i wasnt charged a dime for the form to be completed and signed..

Just quick question though, do i require the entire I-693 completing becasue of this missed shot? or is the vaccination section sufficent for the requirement of the AOS?

There's this person who does all the immigration stuff for her when you go in to have your paperwork transcribed, they will bill you as having the full medical exam and if you call the guy who does the paperwork, he's really mean and awful, who nags worse than any PMSing female!!!

I got ripped off with that, I called him and asked why they billed us just for transcribing, he was attacking me just so I wouldn't prove he's wrong, all he did was nag and hung up..

Dr. Williams is lovely though but her personnel is awful! I don't know if that could be her husband
