K1 and Vaccination records; DS-3025

Please help me understand....We just got an approved k1, she hasn't entered the USA yet. My fiancee tells me she doesn't have a vaccination history, as she's gone to a lot of different doctors in Mexico.

I've read about completing a 693 form and DS-3025 vaccination record. Are we supposed to have the 3025 or is that in the package that we give to customs at the POE?

Will we get a copy to get the rest of the needed vaccinations?

Do those have to be done by specific doctors?

She will need the DS 3025 to adjust status when she gets to the US.

K1 denied, K3/K4, CR-1/CR-2, AOS, ROC, Adoption, US citizenship and dual citizenship

Timeline

To get a copy of the vaccination record she should have gotten one or requested a copy from the civil surgeon that she visited for the k-1 medical.

She can have a titer test to figure out what she is immune to and from that the new civil surgeon in america can decifer what shots if any she needs to complete to be able to file for AOS.

AOS was approved in 2 months and 8 days without interview.

ROC was approved in 3 months and 2 days without interview.

Your family doctor can administer any required vaccinations, but the vaccination record must be given to a USCIS-designated civil surgeon to be officially recorded on the vaccination section of the I-693.

Improved USCIS Form G-325A (Biographic Information)

Form field input font changed to allow entry of dates in the specified format and to provide more space for addresses and employment history. This is the 6/12/09 version of the form; the current version is 8/8/11, but previous versions are accepted per the USCIS forms page.

DS-3025 is not something you get from the embassy. It should be given to you after the medical exam by the doctor. It may also be called DS-2053.
