Last year I did not make above the poverty line.

I am therefore going to have my mom co sponsor with in addition to my financial information.

What form should she fill out. Will she fill out the I-134 like me?

What are you filing for, where in the process are you. Your info says K3, your posting in a K1 group and you have no timeline.

Aug. 27th is the interview

Yes at the consulate stage of the K1 process you and the co-sponsor BOTH will need to fill out the I-134. Just follow the guides and example form for this document and you can't go wrong. :)
